This is a brand new OEM Denso (Koito) HID replacement ballast, which is widely used on newer models of Toyota, LEXUS, MAZDA, etc... as factory OEM ballast. 

This genuine Denso ballast is made in Japan to be used with D4S or D4R Mercury-Free HID Bulb. 
Dimension of the ballast is appx. 11.5 x 10.5 x 1.8 cm, with 7" long cable and it comes with 1 year of limited warranty. 
Please note that the ballast has OEM D4 connectors at the end. 

The Toyota part# for this ballast is '85967-52020' (Lexus may use different part#, the part# may be different).

As you can see in our picture, the ballast has very slim design with a built-in ignitor inside the ballast unit.

    Perfect fit in my 2008 Toyota Prius. Ballast has made headlights flicker for over a year. Finally changed 1 ballast on driver side after it went out and has been working perfectly since. Its been 1 week and hopefully they last

    It worked although the cord is too short for a Prius - needs mod. I'm still happy since I saved over $300 compared to what dealer quoted me.
